Senior COMMUNITY Dietitian, BAND 7 to cover HEF patients, housebound and care homes Monday to Friday, full time 8-4 but flexibility with clinics and would accept part-time also Carrying out functional assessments and interventions with patients who require nutritional support and people on home enteral feeding. This may involve patients in their own homes (housebound only ), HEF patients and also care homes. This may be done either face-to-face or by telephone contact. HCPC registered Dietitian.
